The active in a nappy cream is zinc oxide, and the number next to it is the whole product. This one is 15.0 percent by recipe and assays 15.2 percent by EDTA complexometric titration on the finished cream. That figure is worth printing because the permitted range for zinc oxide as a skin protectant runs from 1 percent to 25 percent. So contains zinc oxide is a legal statement at 1 percent, where it forms no continuous film and does nothing much, and a great many nappy creams sit in the low single figures. Under-dosing is the cheat in this category and it is invisible from the front of the tube. Fifteen percent in a thick anhydrous base gives an opaque layer you can see on the skin, which is the only field test a parent has: if you cannot see it, it is not a barrier. The second thing zinc oxide brings is the reason for a test nobody expects. Zinc oxide is a mined and smelted material, and lead and cadmium travel with it. So the grade is specified as IP and every lot is run by ICP-MS for lead, arsenic, mercury and cadmium against the Cosmetics Rules 2020 limits. Particle size is measured rather than asserted: median 320 nm by laser diffraction with under 1 percent of particles below 100 nm by number, so the word non-nano here is a measurement. And there is no preservative in it, which is a claim that needs a number too. The recipe carries no water phase at all, including the calendula, which is an oil macerate rather than an aqueous extract. Water activity measures 0.32. Nothing microbial grows below 0.60, so the absence of a preservative here is a consequence of the formula rather than a decision to leave one out.

- Honest trade-off one: it is thick and it is meant to be. Spreading it takes effort and it does not vanish into the skin, because a barrier that sinks in has stopped being a barrier
- Honest trade-off two, and this corrects what we used to say: a zinc and beeswax barrier does build up in cloth nappies and reduces how much they absorb. Use a disposable or fleece liner with cloth. Stripping the build-up afterwards is a chore nobody should be signed up for by accident
- Honest trade-off three: an anhydrous cream cannot grow anything on its own, but a finger dipped in eight times a day introduces what a finger carries. Use a clean finger or a spatula, and finish an opened tub within 12 months
- When a barrier cream is the wrong answer: if the rash is bright red with small satellite spots and sits deep in the skin creases, that is likely a yeast, and no thickness of zinc will clear it. It needs an antifungal from a doctor. If redness does not settle in two or three days, or spreads, see your paediatrician rather than reaching for a thicker cream

Using it, and when it is the wrong answer
- Apply
- A visible layer on clean, dry skin at each change
- Next change
- Wipe the soiled part and top it up. Do not scrub it all off, which hurts more than the rash does
- Cloth nappies
- Use a disposable or fleece liner. A zinc and beeswax barrier builds up in cloth and reduces absorbency, and stripping it out afterwards is real work
- Keep it clean
- Clean finger or a spatula. The cream cannot grow anything on its own; a finger can put something in it
- When it will not work
- A bright red rash with small satellite spots deep in the creases is likely a yeast. No thickness of zinc clears that. It needs an antifungal from a doctor
